The Eastern Upper Peninsula Intermediate School District (EUPISD), under the leadership of Superintendent Dr. Angie McArthur and Director of Special Education Rachel Fuerer, has received a grant to explore and implement day treatment solutions that support youth mental health in our region. As a valued community partner, your voice is essential. To better understand current challenges and identify the resources needed to support the mental health of our youth, we are conducting a community needs assessment utilizing these surveys.
There are two versions of the survey:
One for families who have children living in their home: https://eupisd.typeform.com/MentalHealthFam
One for the general community and business sector: https://eupisd.typeform.com/MentalHealthCom
Please complete the survey that applies to you — or both, if relevant. Your responses will remain anonymous; we do not ask for information that would identify you. If you are unable to complete the survey questions all in one sitting, you can go back in as long as you are using the same device and are within 15 days of when you started the survey.
The surveys will remain open until September 15, 2025. Your input will help guide meaningful mental health solutions for youth across the Eastern Upper Peninsula.
