TRANSITION IN THE IEP
Transition Planning Guides: These checklist documents can be used to identify areas of need in each of the four core transition areas (career/employment, training/education, community involvement, and independent living). The areas of need could become activities within the IEP transition page. Utilizing the multi-year documents, student progress can be clearly documented.

Summary of Performance
Vocational Evaluation - a set of formal or informal assessments designed to assess the interests and aptitudes of transition age students. The following formal assessments are available for check out from the EUPISD to use in the completion of a vocational evaluation: OASIS-3: Aptitude Survey, OASIS-3: Interest Survey, Transition Planning Guide-2, ARC Self-Determination Survey, and ESTR-III. Summary of Performance - a summary of academic achievement and functional performance that includes recommendations on how to assist the student in meeting his/her post secondary goals. See the Summary of Performance page for more information.
